Class V
(membranous LN)
Pure membranous: HCQ + RAAS blockade. If nephrotic-range proteinuria persists → MMF 2–3 g/day + low-dose prednisone. Calcineurin inhibitor (tacrolimus or voclosporin) is increasingly preferred per KDIGO 2024. Pure Class V is uncommon in Filipinos (~10% of LN). When seen, tacrolimus 0.05–0.1 mg/kg/day BID is more accessible than voclosporin and has comparable efficacy.
Mixed Class III/V or IV/V Treat as proliferative — same induction as Class III/IV with steroid + MMF (or CYC) + consider triple therapy from the start. Often missed when biopsy is read as "predominantly Class V." Always look at the LM, IF, and EM together.
Class VI
(advanced sclerotic, >90% globally sclerotic)
Supportive care; immunosuppression does not reverse damage. Prepare for KRT. Often a missed Class IV that progressed unrecognized. Highest mortality cohort.

  • Hydroxychloroquine 5 mg/kg/day (max 400 mg) — reduces flares, improves renal response, reduces thrombotic risk. Annual ophthalmology screening after year 5.

Refractory and Relapsing Lupus Nephritis

Approximately 20–30% of LN patients fail first-line induction (no PRR by 6 months, or no CRR by 12 months). The cause is roughly equally split between true drug failure, undertreatment due to dose reduction or non-adherence, and chronic damage misdiagnosed as active disease. The decision tree must distinguish these before escalation.

Step 1 — Confirm true failure (not undertreatment or non-adherence)

The Case for Repeat Biopsy — When and Why

Repeat (per-protocol or for-cause) kidney biopsy in LN remains one of the most underused diagnostic tools in PH nephrology. Its yield is high enough that KDIGO 2024 explicitly endorses it in three scenarios.

Three indications for repeat biopsy
1
Refractory disease at 6 months — if no PRR despite confirmed adherence and adequate dosing. Repeat biopsy reclassifies up to 30% of cases (e.g., from "active Class IV" to "predominantly chronic damage with limited active inflammation" — changing the management entirely).

Renal flare after sustained remission — distinguishes recurrent active disease (escalate immunosuppression) from chronic damage progression with proteinuria (escalate RAAS, do not increase IS).
3
Per-protocol after 18–24 months of remission — increasingly endorsed. About 30% of clinically silent patients show ongoing histologic activity ("silent active disease"); withdrawing IS in these patients carries high flare risk.

Practical PH considerations: ultrasound-guided percutaneous biopsy at PGH, NKTI, St. Luke's, MMC, and most regional tertiary centers is now routine with <1% major complication rate. Cost is approximately ₱8,000–25,000 depending on facility. Coordinate with renal pathology (NKTI, PGH, and a small number of private labs offer immunofluorescence and electron microscopy) — without IF and EM, LN classification is unreliable.

Special Populations — Pregnancy, Pediatric, Transplant

Pregnancy in LN

The single most important rule: plan the pregnancy around remission, not the other way around. Preconception counseling should aim for ≥6 months of stable CRR before conception. Active LN at conception triples the rates of pre-eclampsia, preterm delivery, and fetal loss.

Pediatric and adolescent LN

Pediatric SLE (onset <18 years) carries higher LN prevalence (~80% will develop LN) and worse renal outcomes than adult-onset disease. Filipino pediatric LN cohorts show a particularly high rate of Class IV with crescents at presentation.

LN in dialysis and post-transplant

Once LN reaches ESKD, immunosuppression is generally minimized — clinical SLE activity often quiets dramatically on dialysis. Maintain hydroxychloroquine; minimize chronic steroids unless extrarenal disease demands.

Transplantation: LN is a good kidney transplant indication. Outcomes mirror non-LN ESKD patients when lupus is in remission ≥6–12 months pre-transplant. Recurrent LN in the allograft is uncommon (~10%) and usually mild. Living-donor transplant from a sibling carries a small concern about shared genetic predisposition but is not a contraindication.

Emerging Research and the Future of LN Therapy

The therapeutic landscape for LN has expanded more in the last 5 years than in the previous 30. Below is a clinician's-eye summary of the agents and concepts most likely to reach Philippine practice in the next 1–5 years, organized by mechanism.

Pre-emptive Immunosuppression for Asymptomatic Serologic Reactivation — RCT Commentary

One of the most clinically vexing decisions in lupus nephritis management is what to do when a patient in stable clinical remission shows serologic reactivation — rising anti-dsDNA or falling complement — but remains symptom-free. A landmark 2026 multicenter RCT has now provided the strongest prospective evidence to date on this question.

Population: 49 patients with biopsy-proven Class III, IV, or V LN who achieved complete or partial renal remission and were receiving low-dose maintenance immunosuppression (prednisolone ≤7.5 mg/d with MMF <1.5 g/d or AZA <100 mg/d), then developed asymptomatic serologic reactivation. Recruited from three Hong Kong Hospital Authority centers (2019–2021).

Asymptomatic serologic reactivation was defined as the absence of any renal or systemic SLE manifestations, plus either: (a) anti-dsDNA rising from negative to >100 IU/ml, or (b) a 2-fold increase in anti-dsDNA to >100 IU/ml — with or without subnormal complement.

Pre-emptive Treatment Group (n=24)

Moderate escalation of prednisolone + antimetabolite, followed by structured 12-week taper back to baseline. Detailed schedule below.

Observant Management / Control (n=25)

No change in immunosuppression. Monitored at 4 weeks, 12 weeks, then every 3 months, with prompt rescue treatment if clinical flare occurred.

The PT protocol was designed to use approximately half the immunosuppression dose of a full LN flare treatment — a "moderate escalation" strategy. Both prednisolone and the antimetabolite were escalated simultaneously.

Important Protocol Note

At baseline in the trial, the prednisolone dose was ≤7.5 mg/d. The protocol language states escalation to 0.4–0.5 mg/kg/d from "≤5 mg/d" — meaning many patients had prednisolone escalated from 5 mg to roughly 20–25 mg (mean escalated dose in the trial: 23.8 ± 4.2 mg/d). This is substantially lower than the 0.5–0.8 mg/kg/d used for treating full LN flares. The antimetabolite escalation was also modest — many patients were already on MMF 1 g/d and the target was 1.5 g/d, not the 2–3 g/d used in induction.

Commentary — Clinical Implications

This RCT fills a genuine gap in the LN evidence base. Current KDIGO 2024 and ACR 2024 guidelines acknowledge asymptomatic serologic reactivation as a clinical challenge but provide no specific recommendation on whether to treat pre-emptively — the question has simply lacked randomized data. The Yap et al. trial now offers the first RCT evidence that a structured, moderate escalation protocol can prevent renal flares in this setting without incurring unacceptable toxicity.

Strengths of the trial

  • Prospective multicenter design with block randomization — addresses the selection bias inherent in prior retrospective analyses
  • Clear, actionable definition of serologic reactivation (anti-dsDNA >100 IU/ml with or without subnormal complement) based on routinely available labs
  • 24-month follow-up with structured monitoring at 4, 12, and then every 3 months — mirrors real-world nephrology practice
  • PT regimen uses only conventional agents (prednisolone + MMF or AZA) with no expensive biologics — highly applicable to Philippine practice
  • Despite enrolling fewer patients than the original sample size estimate (due to COVID-19), statistically significant differences were still achieved in all primary and secondary outcomes
  • Dropout and loss-to-follow-up rates were negligible across 24 months, supporting protocol feasibility

Limitations to weigh before applying

  • Small sample size (n=49 total): statistical power was adequate for the primary endpoint, but the study is underpowered to detect differences in rare adverse events (e.g., serious infections, malignancy) or in eGFR slope over time
  • Ethnically homogeneous cohort — all Chinese patients: SLE and LN have well-established ethnic differences in disease severity, HLA genetics, and drug metabolism. Generalizability to Filipino patients (who share some — but not all — genetic risk architecture with southern Chinese populations) is plausible but not yet proven
  • Short follow-up (24 months): the benefit in flare prevention may translate to better long-term kidney survival, but this must be demonstrated in future studies. Hard outcomes (eGFR, ESKD) did not differ significantly within the observation window
  • Single ethnic group limits generalizability to other high-LN-burden populations (South Asian, Black, Hispanic)
  • No protocol biopsy at enrollment: subclinical active nephritis at randomization cannot be excluded, though the 6-month median time to flare in controls makes baseline subclinical activity an unlikely major confounder
  • Novel biologics not tested: the authors note belimumab (with its favorable tolerability profile) as a candidate for future PT regimen trials. Whether biologics outperform the conventional MMF/AZA + steroid PT protocol here is unknown

Philippine Practice Perspective

The PT protocol tested here is directly applicable to Filipino nephrology practice: prednisolone, MMF, and AZA are all generically available and PhilHealth-covered at tertiary centers. The serologic monitoring parameters (anti-dsDNA by ELISA or equivalent, serum C3) are available in most metropolitan laboratories, though access is more limited in provincial settings.

A critical prerequisite before applying this protocol in any patient is confirming that the serologic rise is indeed asymptomatic — meaning a careful review for subtle renal activity (rising PCR, hematuria, or 15% creatinine increase from baseline). In the Filipino patient population, where follow-up gaps are common due to distance and cost, a serologic rise may sometimes represent an early relapse that has already begun rather than a truly asymptomatic reactivation. Clinical judgment remains essential.

The trial also reinforces the importance of slow, deliberate immunosuppression tapering during LN maintenance — a discipline that Filipino patients (who often self-reduce steroid doses due to side effects or cost) may not consistently maintain. The mean 8+ years from last LN flare to serologic reactivation in this cohort reflects the very gradual taper schedule practiced at the Hong Kong centers; this extended remission duration may not be typical of patients with poor adherence or inconsistent follow-up.

Take-home message for Philippine LN practice

When a clinically stable LN patient (complete or partial renal remission) on maintenance prednisolone ≤7.5 mg/d + MMF or AZA develops asymptomatic anti-dsDNA rise (>100 IU/ml or 2-fold increase) without any renal or extrarenal manifestations, this RCT supports a pre-emptive moderate escalation of prednisolone to 0.4–0.5 mg/kg/d (max 30 mg) with concurrent antimetabolite dose increase, followed by a structured 12-week taper back to the original maintenance dose. The NNT of 5 to prevent one renal flare, and NNT of 2 to prevent any flare, is clinically meaningful given the long-term renal consequences of repeated LN flares. This approach should be individualized — weighing infection risk, metabolic comorbidities, and patient adherence — and does not yet have guideline endorsement (KDIGO 2024 predates this RCT). An update to KDIGO guidance incorporating this trial is expected in forthcoming revisions.

SELENA-SLEDAI (Petri et al., Arthritis Rheum 2005) — Same 24 descriptors as SLEDAI-2K, plus the Physician Global Assessment (PGA, 0–3 VAS). Used in BLISS-LN, REGENCY, AURORA, and most biologics trials. SELENA flare = SLEDAI increase ≥3 + PGA increase ≥1, or new high-dose steroids/immunosuppressants/hospitalization.
